TGH Care Coordination Center is powered by a robust partnership with GE Healthcare and enhanced by Palantir’s advanced AI platform. It operates 24/7, using real-time data and predictive analytics to streamline patient flow, reduce inefficiencies, and improve care transitions. From the moment a patient enters our system to the time they are discharged, C3 ensures every step of their journey is coordinated, efficient, and safe.

Real-Time Tools That Drive Real Results

C3 leverages over 20 AI-powered applications—known as “Tiles”—to provide actionable insights to care teams throughout the hospital. These include:

  • Patient Manager – Tracks patient status and needs in real time
  • Capacity Snapshot – Provides a live view of hospital bed availability
  • Discharge Barriers – Identifies and removes obstacles to timely discharge
  • Imaging Expediter – Accelerates diagnostic workflows
  • Surgical Tube Map – Optimizes surgical scheduling and throughput

These tools, combined with a digital twin of our hospital’s patient flow, have enabled us to reallocate resources, reduce bottlenecks, and enhance operational performance.
