Tampa General Hospital’s Release of Information Department is available to assist with your medical records request. We serve patients of our hospital-based inpatient and outpatient facilities, including TGH Davis Islands, TGH Brooksville, TGH Spring Hill, TGH Crystal River, and other locations throughout Hillsborough County, the Tampa Bay area, and surrounding communities in Citrus, Hernando, Pasco, and Pinellas counties. Support is also available for all Tampa General Medical Group locations, fully owned Tampa General Provider Network practices such as TGH General Surgeons of the Palm Beaches, and our Ambulatory Surgery Centers, including TGH Outpatient Center, TGH Surgery Center Brandon, TGH Surgery Center at Morsani, and other TGH-operated surgery centers.
Request a Formal Copy of Your Health Record in MyChart
Did you know your active MyChart account provides greater access and control of your health information? You have access to see results as soon as they are available, as well as complete access to your clinical notes. This includes documentation related to your office visits, hospital visits, and procedures. Requests for additional information from your medical records can also be requested and received within your active MyChart account by submitting a “Request formal copy of health record” form from the Sharing Hub.

For questions regarding requesting records, please call (813) 844-7533. Customer Service Representatives are available Monday through Friday between 9 a.m. and 4:00 p.m. and closed daily for lunch from 12 p.m. to 1 p.m.
Requesting Copies of Your Personal Medical Record
All requests for medical records must be in writing. Patients may request their medical records directly via MyChart. If you do not have a MyChart account, request an activation code here to get started. You may also obtain a copy of your medical record by printing and completing the Request for Access to Protected Health Information by Individual Patients form below. The completed request form can be emailed, faxed, or mailed to our office. Instructions are provided on page 2. In most cases there is a charge for copies of your medical record. You will be informed of the charge before copies are provided.
There is no charge if records are released directly to your MyChart account.
There is no charge if records are faxed or mailed directly to a verified physician’s office.

Requesting Copies of Another Individual's Medical Record
Third parties must complete and submit the Authorization to Disclose Health Information form to the TGH Health Information Management department. If you have questions about the disclosure of health information, please contact the Health Information Management Department, Release of Information section at (813) 844-7533.
Amendments/Corrections
Patients may submit an amendment request via MyChart. You may also request an amendment or change to your medical record by completing a Request for Amendment/Correction of Health Information form and submitting it to the TGH Health Information Management Department. Requesting/Revoking A Restriction
Patients may submit a request for restrictions on how your protected health information may be used and/or disclosed to carry out treatment, payment or healthcare operations by completing a Request for Restriction(s) form and submitting it to the TGH Health Information Management Department.
