How to Participate or Enroll
To become a participating or enrolled provider:
Complete and submit the Provider Data Sheet
Our Program Support Specialist will contact you with next steps.
- Participating Providers can access teleconsultation services and skill-building sessions.
- Enrolled Providers (via Memorandum of Understanding) are guaranteed access to consultative services
Note: There is no cost to providers or practices, only a time investment in consultation, collaboration, and training.

Submit your referral along with the provider information through our secure digital Referral Form
To request consult services please have the patient/family complete the following:
- Referral Packet
- Patient Consent and Authorization
- Patient Release of Information
Please include any additional screens administered, as well as any relevant clinical documentation. Also please note that the Hub may request further assessments if needed.

- Use refxxx in Epic to submit a referral for Behavioral Health Hub
- Refer to the TGH Internal Provider Referral Packet.
