Pancreas Transplant Center: Becoming a Patient

Tampa General Hospital’s Pancreas Transplant Program takes a comprehensive, multidisciplinary approach to transplant care for adult patients. Each patient’s treatment plan is coordinated among several medical professionals, including:

  • The patient’s referring nephrologist, endocrinologist, or dialysis team
  • Our pancreas transplant team
  • Our financial counselors
  • Physicians from other hospital departments at Tampa General

When considering your candidacy for our pancreas transplant program, we will take into account your completed pancreas transplant referral form, which your referring physician must use to provide us with your medical history, progress notes, lab reports, procedure reports, and insurance information. We ask that referring physicians provide the most recent records they have, preferably those from the last six to 12 months.

We recommend that you be referred in the earlier stages of your disease. After receiving your referral and discussing evaluation dates with your referring physician, we will meet you at Tampa General Medical Group’s Transplant & Specialty Services location at 409 Bayshore Blvd., or elsewhere if an inpatient evaluation is necessary. During this initial visit, we’ll consider several factors that will help us determine if a pancreas transplant is the best treatment course at this time. Ultimately, our Medical Review Board will decide if it would be best to list you immediately or if it would be better to monitor your condition and admit you to the program in the future.

If you are admitted to our pancreas transplant program, your transplant coordinator will provide you with a comprehensive new patient packet, which contains everything you need to get started. This packet will include:

  • A transplant candidate registration request form
  • A consent form
  • A general patient information pamphlet
  • A medical history survey
  • A map and directions to our facility
  • Additional information about area hotels that offer discounts to our transplant patients
