Becoming a Patient in Our Kidney Transplant Program
Tampa General Hospital’s Kidney Transplant Program is one of the busiest in the nation and provides comprehensive, interdisciplinary care for adult and pediatric kidney transplant patients. To be accepted into the program, you will need a referral from your nephrologist or dialysis center physician, who should send us your patient history and physical exam results, insurance information, progress notes, procedure reports, and lab results. All of the records should be current from the past six to 12 months.
We recommend that patients be referred to our kidney transplant program in the early stage of their disease. When we receive your kidney transplant referral form, our staff will schedule an initial visit at Tampa General Medical Group’s Transplant & Specialty Services location at 409 Bayshore Blvd. to determine if a kidney transplant is an appropriate therapy for your condition. Our staff will also:
Coordinate communication among all parties involved, including you, your physician, the kidney transplant health care team, and other departments within the hospital.
Work alongside TGH’s financial counselors and your insurance provider to obtain insurance coverage for your evaluation and kidney transplant operation.
end you a new patient packet after your referral has been reviewed and accepted. This packet includes a map to our facility, a list of area hotels offering discounts, and general patient information, as well as a medical history survey, transplant candidate request form, and consent form.
